Recommendation Forms Instructions and Downloads

As part of the GMC Prep application process, we require a recommendation from a current teacher for each prospective student. We appreciate your cooperation in completing this form. Please follow the steps below to submit a recommendation form. If you encounter any issues, contact craburn@gmc.edu .

Locate the appropriate form below that correlates with your subject area and download the form.

Choose one of the following steps:

Option 1

Download and complete the form electronically using Adobe Acrobat

Save as “Student Last Name, First Name – TeacherRec” to your computer

Attach and email document to address on the form

Please include the Student’s Last name in the Subject Line

Option 2

Download and complete the form by hand

Scan the document with a scanner or smartphone

Attach and email document to address on the form
